Elsewhere, Shinwell notices something odd about the injured patients in UHOP. After being rejected by his superior, he approaches John, and they learn that the storm might have caused a fungal infection outbreak. There are four patients whose lives are in danger, and the team tries to save them.

Watson Season 2 Episode 4 recap: Shinwell finds out about the outbreak
In Episode 4 of Watson Season 2, the hospital is overcrowded with people after a storm in the area. This made Shinwell, who is a trainee being the busiest among the team. With him handling the patients firsthand, he also becomes the first one who notice that all the patients with injuries have something in common. Their injuries were turning black with oozing pus.
After his superior refused to look into it, he went to John, and just by looking at the pictures, they decided to shut down the hospital due to the chance of having a flesh-eating bacterial outbreak. They began testing everyone in the hospital, starting with Shinwell, who treated the three patients who had the infection. John also learned that this was not just a problem in UHOP but in other hospitals as well, and might be because of the storm.
Watson Season 2 Episode 4 recap: John and the team begin treating the patients with the bacterial infection
John and the team discovered that the tornado had spread some kind of spores into the area, which caused this outbreak. They found four patients with the infection in Episode 4 of Watson Season 2. One of them was a young boy who was about to go to college and had received five basketball scholarships to choose from.
However, he was to lose his leg due to the infection, changing his future completely. Shinwell, who grew a soft spot for him, was with him throughout his treatment, supporting him.
The other critical patient was a woman named Frankie, who had a cut on her thigh. Apart from the infection, she was also in contact with animal spores due to the guinea pigs that the deacon from the church, with whom she was having an affair, had. The bacterial infection was made worse by the flea bites.
John warned them that their messy situation might become public knowledge when they begin the treatment for their infections, but the deacon assured him that Frankie's husband was aware of their relationship.

Apart from the fungal infection outbreak, in Episode 4 of Watson Season 2, Adam learns about his fiancée's pregnancy. Although they had discussed kids before, he confesses to his twin brother Stephens that he might not want kids altogether.
Elsewhere, Shinwell also feels guilty about the boy losing his leg, as he thinks he could have saved him from going through it by taking swift action when he first met him, as he knew something was wrong, but still took longer to act on it and go to John.
John also realises that he might not be ready to be all in with his new relationship with Laila, as he was still thinking about Mary and their marriage, which was evident from the first episode due to his being happy about the storm, which reminded him of the happier times he spent with Mary.
